The deadline is approaching to add the name of loved ones to a 9/11 memorial. 

The Catholic Cemeteries of Long Island is offering families of victims and first responders the option as part of the 20th anniversary of the attack.

Those who lost their lives later due to health conditions and exposure are also eligible. 

All forms must be submitted by Saturday.
