Police say one person has died following a chain-reaction crash in Patchogue.

The incident happened around 9 p.m. Wednesday on North Ocean Avenue.

According to police, the drive of a Cadillac crossed into oncoming traffic.

Investigators say he then hit a guardrail, side-swiped two other cars, then slammed head-on into a Ford Explorer.

The Explorer then crashed into another vehicle.

The 54-year-old man driving the Cadillac was rushed to hospital, but didn't survive.   The other drivers were not seriously hurt.
