A fire forced an evacuation from a nursing and rehabilitation center in Greenport.

The incident happened at San Simeon by the Sound around 4:30 a.m. Monday.

All residents had to be transferred to different locations, according to police and fire officials.

Originally, just 16 of the nearly 120 residents were evacuated.

Later in the day, the NYS Department of Health said it was unsafe for anyone to stay there.

Southold police and Suffolk Fire, Rescue and Emergency Services assisted in relocating people to alternate care facilities.

The fire was caused by a malfunctioning air handler in the HVAC system, police say.

No one was hurt.
