Amanda Burke will be formally charged for allegedly mishandling the 2-day-old child in the Neonatal Intensive Care Unit (NICU).
News 12 first showed the video of the incident that the baby's father captured on his phone in February.
Burke, 29, was fired from the West Islip hospital the same day.
She surrendered to police and was charged with endangering the welfare of a child in April.
Suffolk District Attorney Ray Tierney says Burke still has a license to practice as a registered nurse.
