Officials are investigating after a 26-year-old was found with a gunshot wound to the arm and chest.

They say the incident happened just before 12:30 p.m. today in front of a home on East 163rd Street.

Authorities say Jonathan Santana was transported to Lincoln Hospital where he was pronounced dead.
